I have imported a phq-9 questionnaire as a Questionnaire resource and marked it as "Active". However, when I go to PlanDefinition/builder and add a "task with questionnaire" and it asks me to pick a questionnaire, it doesnt load any questionnaires to choose from.

@reshma thats what it was. The exported json for the questionnaire that I pasted in didnt have a "name" and "description"
